Bound By Blood is a side quest within Diablo IV 4. It can be found over at Margrave. Here we can speak with Lacthan. One of the many villagers. He appears to be searching for his brother. Who was a victim to a bandit attack.
We are then tasked with finding this brother.

DIABLO 4 BOUND BY BLOOD
Head to Margrave and find and speak with Lacthan. He will then inform us about his missing brother, who was victim to a bandit attack. Lacthan will then accept to accompany us in trying to find him.
We can begin our search for the brother (Perithan) by speaking to the locals, including Priest Matvey, Meridan and Guard Cvetko. Neither of them will give us much information but they do regard Perithan as some kind of trouble maker.
However, as he ventured out with the merchants that were attacked recently, there is a good chance that he was part of this group. Our next destination is over to the east.
SEARCH THE AREA FOR CLUES
Head east of Margrave, towards Kroll Forest. Continue east from there and we will arrive at our first destination. There will be many Storm Wargs to confront along the way.
After taking down all of those we will then be able to inspect the area. We can inspect many of the fallen individuals, such as bandits and merchants. As we continue to inspect them we will learn more about what exactly happened here. One of the bandits was apparently killed by an arrow to the neck. There are suspicions that the attack was caused by Margrave hunters.
Remember to also inspect the nearby empty barrels too. In order to learn that the merchant’s goods have been stolen.
WESTERN TUNNELS
After inspecting and finding all of the clues we will then want to inspect the Western Tunnels. There will be a wall at the entrance, simply destroy it using melee or ranged attacks and head inside.
Keep following the path and it will lead to the Cutthroat’s Escape.
CUTTHROAT’S ESCAPE
Continue to explore the area. We will confront new enemies such as the Firebrand and Scorchers. Whilst we battle through the Cutthroat’s Escape we will eventually arrive at a locked door. This door requires the Smuggler’s Key. The location to which will be marked on the map. It can be found amongst the Smuggler’s Pack.
BRING LACTHAN TO PERITHAN
Unlock the gate and continue further in and we will find ourselves at the Smuggler’s Hoard. Continue on into the room where Perithan can be found.
This will trigger the Outlaw Sharpshooter and Lacthan will no longer be accompanying us. The Sharpshooter can create aoe attacks on the ground, do not stand in it. They can also fire projectiles too.
The Outlaw Sharpshooter has the potential to drop the Makeshift Wand and Simple Sickle.
