The Fantasma Trophy is a Mafia The Old Country achievement that requires you to enter Ludovici’s villa without being seen. You can unlock this trophy in Chapter 5, Vendetta Donore.

How To Unlock the Fantasma Trophy in Mafia The Old Country
Keep playing until you reach the Infiltrate Ludovici’s villa objective in Chapter 5. To unlock the Fantasma achievement, you have to make your way to the yellow marker without the guards spotting Enzo. Knocking guards from behind is acceptable. If you alert the guards, restart the checkpoint and give it another go.
How To Enter Ludovici’s Villa Unseen
From the starting point, go to the left of the main entrance to reach a side gate. There will be a single guard behind it. Wait for him to turn around and walk away. Open the gate with the knife, sneak behind the guard, and take him down. Go right, following the yellow marker and stop when you reach the two guards engaged in conversation. You don’t have to knock them down. Distracting them works just fine.
Hide behind the fence and throw a coin to the right. Move to the left and go up the ramp when the guards are not looking. You will reach a ladder. Climb it and open the door to enter the building. The two guards outside are busy checking the car, so ignore them. Open the next door and head outside. Look right to see two guards standing in front of the stairs.
Grab the bottle and throw it to the right of the guards. Sneak past them and take the stairs leading into the villa. Stop and hide, as there will be a guard in front of the entrance. Wait for him to turn and knock him down. Open the entrance door and go left. You will unlock the trophy as soon as you interact with the white door.
Its not really that difficult as long as you are patient and use the nearby bottles and coins for distraction. Remember to loot bodies to collect more resources if necessary.
