Neverness to Everness Nameless Hospital is an Anomaly Commission with four endings. To witness all endings, you need to run it four times and gather clues.

Neverness to Everness Nameless Hospital Endings Guide
The NTE Nameless Hospital quest starts once you speak with the Nameless Client inside the Hethereau Municipal Hospital. Enter the building, go left, and head towards the stairs to find an NPC facing the wall. Interact with the Nameless Client and select the Close Eyes dialogue option to start the quest.
Neverness to Everness Nameless Hospital Ending 1: The Price of Betrayal
From your starting position, head towards the circular desk and investigate the computer to obtain Anomaly Disorder Clue I. Enter the door on the left and turn right twice to reach the vending machines area. Collect Keychain 1F from the reception desk.
Go through the double doors next to the vending machines, follow the corridor, and enter the room on the right. Investigate the computer and collect Anomaly Disorder Clue III.
Enter the room on the left, leave through the open door, and turn left towards the exit door. Keep going until you see a door on the left. Go through this door and investigate the computer to obtain Anomaly Disorder Clue IV.
Leave through the door on the left of the computer desk and go through the next door. Search the locker to find Spare Key 1F.
Destroy the trash bags blocking the door to the left of the lockers and leave the room. Enter the first door on the left and head up the stairs.
Enter through the door behind the reception desk and use the cameras to view the three Anomaly Disorder Clues and unlock the first NTE Nameless Hospital ending, The Price of Betrayal.
Nameless Hospital Ending 2: An Offering to Evil
From your starting position, go past the reception desk and go left. Make another left and you will notice someone opening and entering a room. Follow them into this room and investigate the journal at the back of the room to collect the first Neverness to Everness Nameless Hospital Ending 2 clue, Surveillance Record Clue I.
Return to the corridor and ride the elevator to the second floor (2F). Exit the elevator and make a right to reach the waiting area. Go left and follow the corridor until you spot a neon tube on the floor. Enter the room and investigate the journal to collect Surveillance Record Clue II.
Before leaving the room, switch off the light to reveal the panel holding Keychain 2F. Exit the room, turn right, and enter the first room on the right. Investigate the journal pinned to the board and collect Surveillance Record Clue III.
Make your way back to the room with the three cameras, view the Surveillance Record Clues, and unlock the An Offering to Evil ending.
NTE Nameless Hospital Ending 3: Medical Abomination
From your starting position, make your way to the elevator, and go to the third floor (3F). Immediately after exiting the elevator, search the oxygen tank to collect the Key North District item.
Go through the door next to the oxygen tank and enter the first room on the right. Go through the next door on the right and investigate the journal to obtain Anomaly Experiment Clue II.
Investigate the canister to the right of the journal to collect Jigsaw Type I. Exit the room, turn left, and interact with the jigsaw table.
Defeat the enemies that spawn and go through the door. Take the second right and enter the room to collect the second Neverness to Everness Nameless Hospital Ending 2 clue, Anomaly Experiment Clue III.
Exit the room and return to the jigsaw table. Go through the door and notice the clock on the wall. Interact with it and obtain the Key South District.
Go through the double doors, make a right, and enter the room. Open the cabinet and collect the Keychain Office.
Exit the room, go past the reception desk on the right, go through the door, and enter the first room on the left. Investigate the notes and collect Anomaly Experiment Clue IV.
Return to the room with the cameras and view the Anomaly Experiment Clues to unlock the Medical Abomination ending.
NTE Nameless Hospital Ending 4: Voice of the Nightmare
The fourth Neverness to Everness Nameless Hospital ending is the most complicated. From your starting position, make your way to the elevator, and go to the second floor (2F). Exit the elevator, turn right, and head towards the room where you found the last NTE Nameless Hospital Ending 2 clue, Surveillance Record Clue III. Enter the room and use the water valve. Exit, enter the first room on the left, and collect Keychain 2F from the panel. Exit, go left, turn right twice, and enter the second room on the right. Jump through the hole in the wall to reach the adjacent room.
The next part is tricky. You must jump through the hole in the ceiling. Stand on the IV or on the locker. Use the character’s skill and double jump when you are in midair. Investigate the canister to obtain Jigsaw Type IV.
Jump down, leave the room, and use the elevator to go to the third floor (3F). Exit the elevator, collect the Key North District from the oxygen tank, and use the jigsaw table in the next room to reveal a door. Go through the door and make your way to the back of the room to pass through the wall. Interact with the files on the table four times and obtain the first clue, V???’s Notes.
Return to the jigsaw table, go through the door, and enter the first room on the right. Investigate the canister and collect Jigsaw Type I.
Use the jigsaw table and go through the door. Take the second right and enter the first door on the right. Investigate the notes and collect Anomaly Experiment Clue III.
Exit and enter the room in front of you. Interact with the last canister and collect Misty.
Exit, take the second left, and enter the first room on the right. Interact with the canister and collect Jigsaw Type II.
Return to the jigsaw table and use it. Leave the room, take the second right, and enter the room on the left.
Interact with the switch on the wall using this pattern:
- Two quick taps.
- One tap, wait for one second – repeat four times.
- Two quick taps.
The secret door will open, allowing you to collect Anomaly Experiment Clue VI. Take the elevator to the second floor and return to the viewing room. Use the three clues and unlock the Voice of the Nightmare ending.
