Pathologic 3 Lone Cub is an achievement you can obtain on Day 10. To unlock it, you must make certain choices on Days 3, 4, and 9.

How to Unlock the Pathologic 3 Lone Cub Achievement
The achievement description states that you need to “witness the Haruspex become the last of his circle of friends”. It’s obvious that the Haruspex’s friends need to die, but be aware that all choices come with branching consequences that might lock you out of other Pathologic 3 achievements and deviate from the course of the main objective.
How to Get the Lone Cub Achievement
In order to unlock the Lone Cub achievement, three people must die by the time you start Day 10:
How to Let Rubin Die in Pathologic 3
You have two choices here. You can let the two bandits kill Rubin on Day 3 at the Warehouse. If you already saved him on Day 3, you can let him die on Day 4. On Day 4, Rubin is buried alive. If you don’t make it to the cemetery in time, Rubin dies. However, this means that you cannot interact with him on Day 5. Keep this in mind, as Rubin is a key character for the main objective and A Communion of Eternity achievement.
How to Let Bad Grief Die in Pathologic 3
Bad Grief is another character you must sacrifice in order to unlock the the Lone Cub achievement in Pathologic 3. He dies on Day 4 if you do not save him from the bandits. If you choose this path, don’t approach his hideout in the Warehouse on Day 4.
How to Let Lara Die in Pathologic 3
The Changeling kills Lara on Day 9. You can prevent her death, but if you want the achievement, choose not to intervene when this happens.
With Rubin, Bad Grief, and Lara dead, you have met all the requirements for the Pathologic 3 Lone Cub achievement. All that’s left to do is to attend Lara’s funeral on Day 10. Interact with all the participants and you will unlock the achievement.
